Southwark council putting together a service plan and they want to involve people in shaping the plan
Adult social care services are going to put together a service plan that sets out how we will help and support older people, people with a disability or a physical or mental illness.
To do this we would like to involve people in shaping this plan.
If you are, someone who receives help or support, an unpaid carer or someone who may require help or support, we would really like to hear from you.
