Disability Service Questionnaires

Southwark Council have released three questionnaires (targeted at service users, carers and providers) which will run until 10 April 2022. These questionnaires form part of the engagement plan for the Disability Hub service. It is important that the feedback is representative and that your voices are heard, so please take the time to fill out the questionnaires below and share with your networks and groups.
